Party Payment Form

All Parties over 50 people require a $150 security deposit. This security/cleaning deposit will be refunded if premises are in acceptable condition upon completion of party.

1. Pool rental is available from 12:00 P.M. to 8:00 P.M. The facility IS NOT available for access or setup prior to the designated rental period.
2. Party head count is total number of people in attendance regardless of if they are swimming or not.
3. Once your party has begun there will be no refunds, regardless of inclement weather. Please see cancellation policy below for full details.
4. ALCOHOL IS PROHIBITED ON THE PREMISES FOR RENTALS INVOLVING THE USE OF THE SWIMMING POOL. FAILURE TO COMPLY WITH THIS MAY RESULT IN TERMINATION OF THE EVENT, AND LAW ENFORCEMENT BEING NOTIFIED.

Lessee expressly covenants & agrees as follows:
1. LESSEE will be financially liable for all damage to the premises that occurs during the period of the lease.
2. LESSEE agrees everyone who wishes to use the deep end of the pool (slide and diving boards) will be swim tested.
3. LESSEE agrees any child who is a non-swimmer and wants to wear safety vest, it must be coast guard approved. and an adult must be in water with the child.
4. The LESSEE without the prior approval of the Board of Directors shall undertake no alteration of any nature on the premises.
5. LESSEE agrees to restore the premises to a degree of order and cleanliness it was upon arrival.
6. LESEE agrees that his/her occupancy shall be conducted in such a manner as to not create any nuisance nor interfere with, annoy or disturb neighbors.
7. LESSEE agrees no confetti or glitter will be used on the premises
8. LESSEE agrees and understands that the Board of Directors is the final authority on matters pertaining to the lease and that their decision is final and binding.

Pool Area:
1. LESSEE agrees that it shall not possess any glass containers. If glass containers are found on premises, they will be asked to take items to the car.
2. LESSEE agrees that the premises are suitable for its use and that any injury sustained by LESSEE or their guests occupying the premises shall not be reason or ground for any claim of action against the Maple Valley Swim Club.
4. **FOR ANY VIOLATION THE POLICE WILL BE CALLED FOR ASSISTANCE AND/OR INVESTIGATION.

RESERVATION & CANCELLATION POLICY:
1. Reservations will not be valid until payment of fees and deposits have been received by Maple Valley Swim Club. Reservations are on a first come, first served basis.
2. Lessee may cancel or re-book this lease at any time, prior to 3 days before date of rental. Cancellations or re-bookings occurring less than 3 days before the rental date will forfeit the entire rental charge. The security deposit will not be affected due to cancellation or re-booking.

3. *In The Case of Inclement Weather, the Lifeguard will make the final decision. If the weather is deemed unsafe, the lessee will be refunded the entire rental charge or can select an alternate date.
